McCune–Albright syndrome, described in 1937 by Donovan James McCune and Fuller Albright, is a genetic disorder of bones, skin pigmentation and hormonal problems along with premature puberty.
Contents
Symptoms

It is suspected when two of the three following features are present:

* (autonomous) endocrine hyperfunction such as precocious puberty
* Polyostotic fibrous dysplasia
* Café-au-lait spots

Presentation

Within the syndrome there are bone fractures and deformity of the legs, arms and skull, different pigment patches on the skin, and early puberty with increased rate of growth.

Polyostotic fibrous dysplasia has different levels of severity. For example one child may be entirely healthy with no outward evidence of bone or endocrine problems, enter puberty at close to the normal age and have no unusual skin pigmentation. The complete opposite of that would be children who are diagnosed in early infancy with the obvious bone disease and obvious increased endocrine secretions from several glands.

Approximately 20-30% of fibrous dysplasias are polyostotic and two thirds of patients are polyostotic before the age of ten.

Polyostotic fibrous dysplasia is usually caused by mosaicism for a mutation in a gene called GNAS1 (Guanine Nucleotide binding protein, Alpha Stimulating activity polypeptide 1).

The syndrome shows a broad spectrum of severity. The disease frequently involves the skull and facial bones, pelvis, spine and shoulder girdle. The sites of involvement are the femur (91%), tibia (81%), pelvis (78%), ribs, skull and facial bones (50%), upper extremities, lumbar spine, clavicle, and cervical spine, in decreasing order of frequency. The craniofacial pattern of the disease occurs in 50% of patients with the polyostotic form of fibrous dysplasia.

Sézary syndrome and mycosis fungoides are T-cell lymphomas whose primary manifestation is in the skin. The disease's origin is a peripheral CD4+ T-lymphocyte, although rarer CD8+/CD4- cases have been observed. Epidermotropism by neoplastic CD4+ lymphocytes with the formation of Pautrier's microabscesses is the hallmark sign of the disease. The dominant symptoms of the disease are:

Generalized erythroderma
Lymphadenopathy
Atypical T-cells ("Sézary cells") in the peripheral blood
Hepatosplenomegaly





Dieulafoy's lesion is a medical condition characterized by a large tortuous arteriole in the stomach wall that erodes and bleeds. It can cause gastric hemorrhage but is relatively uncommon. It is thought to cause less than 5% of all gastrointestinal bleeds in adults. It was named after French surgeon Paul Georges Dieulafoy, who described this condition in his paper "Exulceratio simplex: Leçons 1-3" in 1898. It is also called "caliber-persistent artery" or "aneurysm" of gastric vessels. However, unlike most other aneurysms these are thought to be developmental malformations rather than degenerative changes.


Dieulafoy's Lesions are characterized by a single large tortuous arteriole in the submucosa which does not undergo normal branching or a branch with caliber of 1-5 mm (more than 10 times the normal diameter of mucosal capillaries). The lesion bleeds into the gastrointestinal tract through a minute defect in the mucosa which is not a primary ulcer of the mucosa but an erosion likely caused in the submucosal surface by protrusion of the pulsatile arteriole.

Approximately 75% of Dieulafoy's lesions occur in the upper part of the stomach within 6 cm of the gastroesophageal junction, most commonly in the lesser curvature. Extragastric lesions have historically been thought to be uncommon but have been identified more frequently in recent years, likely due to increased awareness of the condition. The duodenum is the most common location (14%) followed by the colon (5%), surgical anastamoses (5%), the jejunum (1%) and the esophagus (1%). The Pathology in these extragastric locations is essentially the same as that of the more common gastric lesion.

Interestingly and in contrast to peptic ulcer disease, a history of alcohol abuse or NSAID use is usually absent in DL.

Symptoms
The symptoms due to bleeding are hematemesis and/or melena, possibly with shock.

Treatment
It is diagnosed and treated endoscopically, however endoscopic ultrasound or angiography can be of benefit.

Endoscopic techniques used in the treatment include epinephrine injection followed by bipolar electrocoagulation, monopolar electrocoagulation, injection sclerotherapy, heater probe, laser photocoagulation, hemoclipping or banding.





Dimorphic fungi are fungi which can exist as mold/hyphal/filamentous form or as yeast. An example is Penicillium marneffei:

* At room temperature, it grows as a mold.
* At body temperature, it grows as a yeast.

Several species are potential pathogens, including Coccidioides immitis, Paracoccidioides brasiliensis, Candida albicans, Ustilago maydis, Blastomyces dermatitidis, Histoplasma capsulatum, and Sporothrix schenckii.

Dimorphic fungi are fungi that can reproduce as either a mycelial or a yeast-like state. Generally the mycelial saprotrophic form grows at 25° C, and the yeast-like pathogenic form at 37° C. Dimorphism is extremely important in the identification of mycoses, as it makes rapid identification of many pathogenic organisms possible. Some diseases caused by dimorphic fungi are

* sporotrichosis
* blastomycosis
* histoplasmosis
* coccidioidomycosis
* paracoccidioidomycosis
* penicillosis

Adalimumab is a completely human IgG1 approved for use in rheumatoid arthritis.

Etanerceptis a dimeric fusion protein composed of human IgG1 constant regions (CH2, CH3, and hinge, but not CH1) fused to the TNF receptor.Etanercept is approved for adult rheumatoid arthritis, polyarticular-course juvenile rheumatoid arthritis, and psoriatic arthritis.

Infliximab is a human-mouse chimeric IgG1 monoclonal antibody possessing human constant (Fc) regions and murine variable regions. Infliximab is currently approved for use in Crohn's disease, ulcerative colitis, rheumatoid arthritis, ankylosing spondylitis, and psoriatic arthritis.

Efalizumab is a recombinant humanized anti-CD11a monoclonal antibody approved for the treatment of adult patients with severe psoriasis.

Omalizumab is an anti-IgE recombinant humanized monoclonal antibody that is approved for the treatment of allergic asthma in adult and adolescent patients whose symptoms are refractory to inhaled corticosteroids .

Eculizumab is a humanized IgG monoclonal antibody that binds the C5 complement component, inhibiting its cleavage into C5a and C5b thereby inhibiting the terminal pore-forming lytic activity of complement. Eculizumab is approved for patients with paroxysmal nocturnal hemoglobinuria .

Palivizumab is a monoclonal antibody that binds to the fusion protein of respiratory syncytial virus, preventing infection in susceptible cells in the airways.
